Introduction
Case NotesCase Notes is a proven Electronic Health Record System (EHR) designed to connect physicians, clinicians, hospitals and other service providers - automating clinical processes and improving patient care. It is in widespread use in the UK and is now available for the first time in North America.

Case Notes provides a complete clinical management environment that can be used across integrated delivery systems; in enterprise or regional health networks; or as the repository at the heart of a centralized health records bank. It is also effective for individual healthcare providers, improving performance, communication and service delivery from the hospital to the practice level.

Case Notes is particularly suited to the management of patient care across multiple locations and organizations. Information and functionality is presented in a clear and familiar way through an easy-to-use web browser interface. Based on open standards, the Case Notes technology platform allows phased migration of data and functionality from existing, legacy systems.

Case Notes can be configured to play several different roles -

  • The core framework for a local, regional or national electronic Health Record Banking (HRB) system, including a patient data repository, access control framework and service-oriented import and export capabilities. In this mode it serves as the central repository for information that is gathered from multiple systems and locations, providing immediate access to a patient’s entire medical history from any connected location.
  • A complete Electronic Health Records (EHR) system for clinical management and patient care within or across hospitals, enterprises and integrated delivery systems. Case Notes’ Hospital Inpatients Package provides tools for managing long-term and acute conditions, including configurable care pathways, integrating easily to existing administrative, departmental or peripheral systems. The Ambulatory Care package adds support for clinics and physicians’ offices.

In addition, Case Notes provides a base for CSW’s Clinical Service Improvement applications including Emergency Medicine Discharge and Assessment (EMDAS) and RealTime BED.
